Personalized evaluations to meet your child’s unique needs
Why evalutation is important
A comprehensive evaluation provides:

Independent evaluation of the young person utilizing standardized, age-appropriate cognitive, academic, behavioral, emotional, and social functioning tests, in addition to clinical interviews.

Written report that documents the presenting concerns and provides a comprehensive and integrated picture of the young person’s functioning across important life domains.

Psychoeducation aimed at helping parents, therapists, and other professionals understand all the factors that underlie and contribute to the young person’s presentation.

Recommendations for therapeutic interventions, home behavior plans, and placement. Parents are able to make informed decisions and advocate for their child or adolescent.

Guidance for educational decision-making including an appropriate academic setting and the implementation of educational supports and accommodations (e.g., 504 and IEP plans).
Fees and Payments
Insurance: Dr. Zeisz is an In-network provider for NC-Blue Cross Blue Shield; otherwise she is considered an Out-of- network provider. Out-of-network services are reimbursed at varying rates based on your family’s plan. She does not submit claims to insurance companies. Instead, she provides a detailed superbill that contains CPT codes and other information necessary to submit a claim to your insurance company for reimbursement directly to you.
Payment: An advance deposit of 50% for psychological evaluations is due prior to the assessment date, with the balance due upon completion of the written report. An Intuit Invoice will be sent to your email address to secure payments.
Dr. Zeisz provides “student scholarships” to families in need of financial assistance.
Other Services: Dr. Zeisz provides counsultations and testimony related to my evaluation/written report when appropriate. These services are billed at a set hourly rate and are not reimbursed by insurance.
